Premier’s Mission statement is as follows:  “To positively impact and improve client’s wellbeing and healthcare outcomes.  Our clients are at the center of all we do.”  Our goals as care managers are to help clients understand their current health status, what they need to do about it and why those treatments are so important.  Our care managers help provide an array of services to help individuals and families cope with complicated situations in the most effective way possible, thereby achieving a better quality of life.  They help people identify their goals, needs, and resources.  From that assessment, the care manager and the client together formulate a plan to meet those goals. 
The care manager helps clients to find resources and facilitates connection with services.  Sometimes the care manager advocates on behalf of a client to obtain needed services.  Care managers also maintain communication with the client to evaluate whether the plan is effective in meeting the client’s goals. In this way the care manager serves as a means to better health care.  Care managers enable their clients to achieve goals more effectively and efficiently. 
The health care system can fail to meet the needs of the elderly population because care is rushed and responsibility is fragmented and the burden is placed on the client and their families in initiating their own follow up care with little understanding of their own conditions or the complexities of today’s health care system.  Care managers focus their efforts on improving the coordination of care among the various care settings to improve patient safety, quality of care and outcomes.
